What is Chronic Disease Management?

Chronic diseases are long-term conditions that require regular monitoring and treatment to manage symptoms and prevent complications. Common chronic conditions include:

• Diabetes

• Heart disease

• Asthma

• Chronic Obstructive Pulmonary Disease (COPD)

• Arthritis

• Osteoporosis

• Chronic kidney disease

• Mental health conditions

Who is Eligible?

If you have a chronic medical condition that has been (or is likely to be) present for six months or more, you may be eligible for a Medicare-funded Chronic Disease Management Plan. These plans may also allow you to access up to five subsidised allied health visits per calendar year.

Our Approach

At our Greensborough clinic, we take a team-based, patient-centred approach to chronic disease care. Our experienced GPs work closely with you to develop a tailored GP Management Plan (GPMP) and, where appropriate, a Team Care Arrangement (TCA) to coordinate with other allied health professionals such as physiotherapists, dietitians, and podiatrists.

We focus on:

• Early diagnosis and prevention

• Regular reviews and health checks

• Medication management and education

• Lifestyle advice including diet, exercise, and mental well-being

• Referral and coordination with specialists or allied health providers

• Support for self-management and goal setting
